TERMS OF
PARTICIPATION
APPLICATION TO
PARTICIPATE
1. By submitting the application form, you are applying for your
child to participate in a TEAMelbourne (TM) Mega Sports Clinic
(Clinic) and are deemed to have accepted these Terms of
Participation.
2. Acceptance of your application is subject to availability of
places. The number of participants in the clinic is limited to
500. In the event that the clinic is oversubscribed,
participant names will be randomly selected from those applications
received by the closing date of 26 September 2008.
3. You warrant that your child is between the ages of 8 and 12
(inclusive) at the time of his or her participation in the Clinic
and agree that you must, if requested by TM, provide evidence of
your child’s age.
4. If you want your child to participate in the Clinic, you
must:
a. complete all fields on the
application form;
b. submit the application form via the
TM website or drop it in the box at the TM booth at any Victorian
Coles supermarket, by no later than 26 September 2008.
5. If your child is selected, TM will send you a letter two
weeks prior to the clinic confirming your child’s
participation. If your child is not selected, TM will contact
you either by email or SMS to advise you of this.
6. TM will not:
a. process any application that does not
comply with clause 2 or which is incorrect, incomplete, unclear or
damaged;
b. accept any changes to the
application, except for a change of contact details.
LIMITATION OF LIABILITY AND
RELEASE
7. You understand that there are inherent risks associated with
your child’s participation in the clinic, which may result in
personal injury (even of a serious nature) to participants and that
you fully accept and agree to bear those risks.
8. You agree not to bring any claim or proceeding against TM for
any damage, loss, injury or liability you or your child may suffer
from participation in the clinic. Except for liability that by law
cannot be excluded, TM excludes all liability to you and your child
in tort (including negligence), contract or bailment for acts or
omissions of TM, its employees and volunteers arising out of or in
relation to the Clinic.
CANCELLATION AND
TERMINATION
9. TM is not liable to you for any loss or damage you suffer as
a result of the Clinic being cancelled, postponed or changed.
10. TM may terminate your child’s involvement in the Clinic
at any time, including if your child’s behaviour interferes with
other participant’s safety or enjoyment of the Clinic
